Description:
Implementation of direct nursing care to assigned patients and their families in the home setting. Case manages the coordination, supervision and direction of all Home Health services.
Requirements:
Graduate from an NLN accredited School of Nursing .
BSN preferred.
Current New Jersey RN License
Current New Jersey drivers license in good standing
Minimum of three years current Home Health experience. Knowledgeable in process improvement, quality studies; State, Federal and JCAHO regulations as they relate to Home Health.
Maintains required BLS Heart Saver/Health Care Provider course completion status.
