Description:
To provide physical therapy services for clients on the Home Health Program. Responsible for instruction and follow through with primary VN, Home Health Aide or caring person in the home, consultation to consultation to other home health professional staff members’. Responsible for equipment used in client’s home necessary for performing physical therapy.
Graduate of a program in physical therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Physical Therapy Association or its equivalent and current State license.
Must have minimum one year experience as a Physical Therapist.
